Complications of taking warfarin: hemorragic syndrome

It is represented two clinical cases of severe haemorragic syndrome caused by uncontrolled taking of warfarin. The manifestations of the syndrome in patients included gastrointestinal and renal bleeding, extensive hematomas, hemorrhagic shock and also DIC. In the first case warfarin was prescribed in high dose by mentalist without any control. In both cases hemorrhagic syndrome and its complications were compensated during first day by administration of fresh frozen plasma (to 3300 ml).

A review of methods influencing to skin microcirculation

The condition of skin microcirculation is one of the relevant objectives which decision will provide significant progress in diagnostics and treatment skin aging. This article reviews modern methods of skin treatment in order to improve microcirculation condition. Non-invasive physical methods of diagnostics of microcirculation condition were analyzed. It is discussed a present-day view of human skin microcirculation specific features normally at a young age and during aging.

Diagnostics of chronic disseminated intravascular coagulation in patients with injured spleen in late postoperative period

Examination of coagulation hemostasis in 70 patients undergone spleen injury as a result of closed intraabdominal injury was carried out. At least one year had passed away after surgical treatment. Age of examined patients ranged from 20 to 50 years with an average age of 45 ± 2 years. There were 50 male patients and 20 female patients, 50 of them underwent splenectomy and 20 – organ preservation surgery with using carbon dioxide laser. The comparison group included 30 relatively healthy people of both sexes of the same age. It was determined that carrying out organ preservation surgeries in spleen injuries prevent disseminated intravascular coagulation, whereas in patients underwent splenectomy the signs of this process were revealed without any clinical findings. It can therefore be said that these patients have chronic latent disseminated intravascular coagulation.

The organization of specialized neurosurgery care to scull injured during military operations under conditions of mountain and gaunt landscape and hot climate of Afghanistan

(By working experience as chief of neurosurgery department of Tashkent district military hospital named after P.F. Borovskii — Neurosergeon-in-chief of Red bannered Turkestan military region) The article discusses questions of organization healthcare delivery at evacuation chain during military operations in Afghanistan to 450 patients with skull injuries representing 5,8 percent of total patient number. Multiple and combined injuries took place at 52,6 % of cases, complicated by shock – 32,6 % of cases.
Peculiarities of the healthcare delivery in mountain and gaunt landscape under exposure of modern shells and severe environmental conditions are shown. Necessity of aeromedical transportation was proved in order to shorten time of wounded delivery to chain and to improve treatment results. Nominally the whole treatment process is divided into 3 periods, either of which differs by improvement of neurosurgical care delivery. The idea of reasonability of various medical care forms at chain in relation to particular battlefield is introduced.

Diagnostics and treatment of chronic venous compartment syndrome in patients with chronic lower limbs vein disease of c5−c6 clinical classes

The article is dedicated to improvement of diagnostics and treatment of chronic lower limbs diseases of C5−C6 clinical classes complicated with chronic venous compartment syndrome. The analysis of parameters of functional electromyography, intrafascial pressure and CT was carried out. It was proved an effectiveness of endoscopic decompression fasciotomy.

Diagnostics of spleen injuries in elder patients

The article studies algorithm of diagnostics spleen injuries in elder patients. Traumatic spleen injuries take one of the leading positions in abdominal surgery. Ruptures of this organ occur in 20−25 % patients with abdominal traumas. Diagnostics of closed spleen injuries quite often causes certain difficulties. It is determined that clinical presentation in elder patients with closed spleen injuries and intraabdominal haemorrhage of 500 ml is characterized by more significant haemodynamic changes in comparison with young and middle age patients. In elder patients with mild or moderate haemorrhage take place more significant haemodynamic changes, however in patients  with severe haemorrhage these changes don’t take place.

He was the first surgeon who used endoscopic method and was one of the founders of coloproctology in Russia.
