Vascular-thrombocytic part of hemostasis in the long-term postoperative period in children underwent spleen surgeries

Study goal: to study correlation between the surgery performed due to spleen injury in children and the changes in vascular-thrombocytic part of hemostasis in the long-term postoperative period. Material and methods. Platelet aggregation activity and their carbohydrate composition were studied in 56 children underwent surgeries on damaged spleen during more than one year. Platelets aggregation activity and their carbohydrate composition were studied in 56 children underwent surgeries on damaged spleen during more than 1 year. Platelets aggregation was studied using a laser aggregation analyzer “Biola 230”. Plant lectins (concanavalin A (Con A), wheat germ lectin (WGA) and phytohemagglutinin P – PHA-P) were used as aggregation inducers for washed platelets. Results. It is shown that splenectomy leads to an increase of platelets aggregation activity in the longterm postoperative period due glycoprotein b-D-galactose, N-acetyl-D-glucosamine and N-acetyl-neuraminic (sialic) acid receptors. Herewith, glycoprotein receptors containing b-D-galactose are more important. Autolientransplantation partially prevents changes in platelets aggregation activity in the long-term postoperative
period. An increase of platelets aggregation activity after autolientransplantation is associated with mannose receptors. In 42,8 % of cases, the changes of platelet aggregation after splenectomy result in the complications.

Experience of ursodeoxycholic acid drug (ursodez) administration for primary biliary cirrhosis patients

The goal of the study was to assess the safety and efficacy of ursodeoxycholic acid drug (UDCA) Ursodez for primary biliary cirrhosis patients (PBC).
Materials and methods. The study included 7 women aged 44 to 71 years with a history of PBC from 2 to 6 years. PBC was confirmed by biochemical, immunological (AMA) and instrumental methods. Ursodez was used as UDCA drug and was administered to each new patient included into the study at a dose of 15 mg / kg / day for 12 weeks. The parameters to be assessed included the clinical symptoms (weakness, itching) from 0 to 5 points, blood biochemical parameters (bilirubin, ALT, AST, GGT, alkaline phosphatase, cholesterol, total protein and its fractions) on the 4th, 8th and 12th weeks. Results. After 12 weeks of Ursodez trea tment, the patients noted decreased weakness; less itching was registered in 6 of 7 patients. Three patients with increased level of bilirubin before Ursodez treatment achieved normal level of this parameter on 12 week. Significant decrease in average GGT level was registered (from 154,3 ± 57,8 U / l to 62,8 ± 15,3 U / L (p < 0,05)). Other cholestasis parameters were not
significantly changed. Decreased activity of the inflammatory process was revealed. Average ALT level decreased from 1,9 ± 0,3N to 1,2 ± 0,1N (p < 0,05). Ursodez tolerability was comparable to that of the other drugs from this group.

Dynamics of plasma antioxidant activity parameters in physiological and complicated i trimester of pregnancy

The article describes the dynamics of ceruloplasmin and total plasma antioxidant activity level in normal course of I trimester of pregnancy and in threatened miscarriage. It was determined that from the pregnancy beginning, the level of blood antioxidant activity increases on 62–81% as compared to the nonpregnant women. In threatened miscarriage the level of ceruloplasmin and total plasma antioxidant activity decreases.

Composition of the carbohydrate component in the platelets glycoprotein receptors in patients underwent surgeries on damaged spleen in the long-term postoperative period

The goal of the study was to analyze the changes in the carbohydrate part of the platelets glycoprotein receptors in patients underwent splenectomy, autolientransplantation and conservative surgeries in the longterm postoperative period. Materials and methods. Changes in the carbohydrate part of the platelets glycoprotein receptors were
analyzed in 126 patients underwent surgery on the damaged spleen during more than one year. Among this group, 26 patients were examined using laser technology after conservative surgeries, 46 patients had autolientransplantations and 54 underwent splenectomy. The control group consisted of 50 relatively healthy people of the same age and sex. Platelet aggregation was assessed by standard turbidimetric analysis. Plant lectins were used as the inducers of platelet aggregation. The analysis revealed that the parameters of the platelets membrane glycoprotein receptors do not correlate with the changes in the platelet membrane glycoprotein receptors in patients underwent conservative surgeries on damaged spleen in the long-term postoperative period. Autolientransplantations patients showed no changes in b-D-galactose and N-glycans platelets membrane glycoprotein receptors. At the same time, the study showed increased aggregation activity for platelets bearing N-acetyl-D-glucosamine and N-acetylneuraminic (sialic) acid glycoprotein receptors. In the splenectomy patients the investigators registered increased aggregation activity of the platelets with b-D-galactose, N-acetyl-D-glucosamine and N-acetylneuraminic (sialic) acid glycoprotein receptors and increased platelets aggregation activity due to increased number of mannose glycoprotein receptors.
